ViaForms is a turnkey forms management application that is hosted and managed by ETI. The features of ViaForms includes the ability to generate forms, fill them out, email them to recipients, sign them electronically, save the forms back to the ViaForms application and access the form data. ViaForms also includes a stand-alone form link technology so users can start forms outside of the ViaForms application and bring the data and transaction management within ViaForms for better management. Definition of Terms

• Application means any system, app or other software product that makes use of the Quik! software to deliver forms to end users
• Customer means the company who is licensed to use Quik!
• Forms means any document output by Quik! (e.g. form, application, contract, agreement, marketing document, etc.)
• User or end-user means any person who is using a form output by the customer's application
