Report Form Errors

While our forms team strives to deliver accurate, perfectly working forms that are always up-to-date, it is possible for you to discover an error with a form that should be reported. To report an error, simply right-click on a form in the search results pane. A pop-up box will prompt you to select a reason and allow you to type a message. This message will be sent directly to our team and someone will respond to you within 24 hours – there is no need to send the message more than once.
